1. This is not an application to the Fund. The application form is available on the Ontario Creates Online Application Portal.

2. Your application will be reviewed for eligibility and completeness after the deadline. It is your responsibility to ensure that your application material clearly demonstrates your eligibility for the program. Information provided via this form and in a consultation call is not considered part of your application.

3. Consultation calls are offered as a courtesy to potential applicants. They are not a replacement for reviewing and understanding the guidelines.
